Request For Quote- Plasma pattern cuts in .032" aluminum sheet

I am looking for a shop that can provide ongoing support in cutting fairly simple patterns is .032" sheet metal. I am attaching a sample to get a sense of prospective costs. Interested shops should provide:

1) Cost for single test prototype
2) Cost for 10, 20, 30 unit runs
3) Approximate turnaround time

Tolerances are not critical for now.
